One other point you touched was about people who have never heard the gospel. One may argue that anyone who has not accepted Christ as personal saviour has not truly "heard" the gospel, which would transfer a lot of responsibility from the hearer to the preacher. But perhaps it is more fair to consider how much divine grace is involved in the process of salvation: sufficient for us who have heard, and perhaps even more sufficient for those who have yet to hear of the love of God.
